Taking the time to create claim kits prior to an accident even taking place will assist you with timely reporting. Refer to our claims kits page for easy packet assembly in order to have the materials on hand in the event of an on the job injury or illness.

Electronic Accident Reporting*
The states of Idaho and Montana allow employers and injured workers to report an accident electronically. To start a new electronic accident report, collect all the required information and then visit our Electronic Accident Reporting Form. State-Specific Accident Reporting Forms
For states that do not offer electronic accident reporting, or if you prefer to view/print forms manually, we offer state-specific workers' compensation forms for all states in our region, including USL&H forms.
*The states of Alaska, California, Oregon and Washington do not support electronic submission of claims.
