For accidents that occur on the job, it is imperative that the workers' compensation claim be reported in a timely manner. For your convenience, we've placed online accident reporting forms on our website.

Claim Kits
Taking the time to create claim kits prior to an accident even taking place will assist you with timely reporting. Refer to our claims kits page for easy packet assembly in order to have the materials on hand in the event of an on the job injury or illness.

Be sure to collect all the required information before submitting your form(s).

Electronic Accident Reporting*
The states of Idaho and Montana allow employers and injured workers to report an accident electronically. To start a new electronic accident report, collect all the required information and then visit our Electronic Accident Reporting Form. If you need help or have any technical questions, please refer to the Help and Technical Requirements page.

State-Specific Accident Reporting Forms
For states that do not offer electronic accident reporting, or if you prefer to view/print forms manually, we offer state-specific workers' compensation forms for all states in our region, including USL&H forms.

*The states of Alaska, California, Oregon and Washington do not support electronic submission of claims.
